Contract of Agreement regarding The Lash Affect Training Course
This course is a technique training, not a license.
Course Agreement:
Lash Affect Training Certification
-Learn lash 101 and safety, proper isolation and placement, lash mapping and styling, ergonomics, volume fanning, and more!
-Receive hands-on training with mannequin practice as well as live model experience
-Upon completion, students receive framed certificate of completion

As stated in The Lash Affect Training Course. I may be awarded the course completion certificate and only at the discretion of the instructor. To be eligible for a complete course a student must have satisfactorily completed a substantial portion of the course. The instructor must complete the full lash training course with the student. Both the student and the instructor must sign the contract, a copy of which is to be given to the student, the instructor retains a copy, a copy is kept on file by the Department Course of Lash Affect. Upon the student’s timely satisfaction of the Course Completion Contract, the instructor will sign and give the student a completion certificate. This form is an agreement between the student and the instructor identifying the work to be completed, and the grade that will be assigned if the work is not completed. (This course is a technique training, not a license)
Payment Agreement:
The payment Agreement for The Lash Affect Lash Training Course of $1,600 is paid in full. If the Student pulls out of the course before the completion of the training, fees are non refundable resulting in the loss of any money paid towards the $1,600. If there is an emergency or reason as to why the training needs to be stopped temporarily, if the instructor is informed about the pause of the course on good terms you may continue the completion of the course within 30 days before the student loses credit, failure to inform the instructor will result in loss of credit. Any money paid towards Lash Affect training course is non refundable (exceptions apply if it applies as stated above).
